What You’ll Do (High Level)
- Build and guide care plans that keep clients safe and supported at home
- Partner with families, physicians, and care teams to keep everyone aligned
- Support and coach field staff so they feel confident and cared for
- Spot problems early and help solve them before they become crises
- Help ensure care stays consistent, covered, and well-coordinated over time
- Keep documentation clear and timely—so care is protected and benefits aren’t delayed

What You Need
- Active RN license
- A steady, compassionate leadership style
- Strong communication and follow-through
- Comfort guiding others and building trust with families
- Home health experience is a plus (but not required for the right nurse)
