## Your Right to Choose Your Care Providers

Your care should feel supportive, comfortable, and centered around you.

Under the Department of Labor (DOL) program, you always have the freedom to choose the providers who feel like the best fit — for Home Health, DME, Respiratory Therapy (RT), Physical Therapy (PT), and any other approved service.

Choosing the right care team is personal, and you deserve the option to select the providers who make you feel safe, respected, and well cared for.

## You Can Change Providers at Any Time

Sometimes your needs change. Sometimes you simply connect better with a different team. Whatever the reason, you may change providers whenever you choose.

Switching providers does not affect your eligibility, your White Card, or the care you are entitled to receive.

You are always in the driver’s seat.

## You Can Use More Than One Provider

Your care does not have to come from just one place.

Many people prefer different providers for different parts of their care — and that’s completely normal and fully supported by the DOL program.

For example:

You can get DME** from one business

-
**Therapy** from another

-
And **Home Health** from a separate provider

You can mix and match however you like.

There is no “all-or-nothing” requirement — **your care can be exactly what you want it to be.**

If a family member is your nurse or home health aide, you still have full freedom to choose the provider you prefer.

Your loved one can continue caring for you regardless of which company you select.

Your decision about providers has no impact on their ability to continue supporting you.

You do not give up any choice just because someone you love is part of your care team — you can choose what makes the most sense for your health and comfort.
